We are experiencing the same issue with. This happens with different Customers, different sites, different environments.
I have managed to replicate this repeatedly by tabbing through multiple windows and copying and pasting.

I will try to replicate this with Sample in a server-client environment.

This is mainly Linux Server and Windows/MacOS clients.
